Who is coming?

We ask this once. After today, booking anything takes a few taps — and there is no password to make.

First name Surname So we can tell two children with the same name apart on the register. Date of birth We use this to work out which classes they are old enough for. Anything we should know Medical conditions, allergies, additional or learning needs, and any injury that limits what they can do. Please say which movements to avoid — for example “scoliosis: no forward rolls in acro”, rather than the condition on its own. Leave it empty if there is nothing; only Miss Kayleigh and the teacher in the room ever see this. Emergency phone number The number we ring if something happens in class. Your own mobile is a perfectly good answer — most families give exactly that. Who to call Their name, and how they know your child — “Sarah Bell, grandmother”. Leave this blank if the number above is your own.

Photographs

Answer what you like, or leave them blank. It makes no difference to the booking, and you can change any of it later on your account.
